Ohio currently has 21 approved symptoms & conditions.

Under Ohio law, all of the following are qualifying medical conditions: AIDS, amyotrophic lateral sclerosis, Alzheimer's disease, cancer, chronic traumatic encephalopathy, Crohn's disease, epilepsy or another seizure disorder, fibromyalgia, glaucoma, hepatitis C, inflammatory bowel disease, multiple sclerosis, pain that is either chronic and severe or intractable, Parkinson's disease, positive status for HIV, post-traumatic stress disorder, sickle cell anemia, spinal cord disease or injury, Tourette's syndrome, traumatic brain injury, and ulcerative colitis.

Additional symptoms are subject to change and may be added to this list at the discretion of the State of Ohio Medical Marijuana Control Program.

Assuming you meet the requirements and possess one of the qualifying symptoms and conditions, the process is very simple and straightforward:

  1. Schedule : Simply select the date, office and available time which works best for you
  2. Register : Once you've scheduled, you'll receive an email to register with OnPatient, follow the link found within the email
  3. Forms : Found within OnPatient, a brief Intake Form will need to be completed along with a HIPPA consent doc
  4. Pay : Once the forms are completed, you will be prompted to pay. We accept all major credit cards.
  5. Consultation : An OGT physician will meet with you to learn more about your symptoms and inform you of the benefits (and risks) of using medical marijuana.
  6. Follow-Up : Ohio law requires a bona-fide patient/physician relationship. To be compliant we will conduct periodic check-ins throughout the year.

The total cost is $199 + tax. This includes the physician consultation and all required follow-ups.

We accept all major credit or debit cards, PayPal, personal checks or cash.

The privacy of our patients is a top priority of ours, so we make sure all patient information is kept confidential in our HIPAA compliant file storage system and EHR.

The short answer, no. But, if you have a qualifying condition or symptom the likelihood is extremely high. Physicians must use the info you provide, medical history and any supporting documentation to provide a suitable recommendation.

In the off chance you are not approved, we will provide a full refund within 3-5 business days.

While this isn't a requirement, helpful supporting documents may include a copy of your medical history, surgeries, prescriptions, prior recommendations or any relevant physician notes regarding your conditions.

Now that you've successfully completed your consultation with one of the OGT doctors, we will compile all of the evaluation notes, medical history and/or supporting documents.

We will need you to sign a patient consent and attestation form. This form outlines that the physician reviewed the benefits and risks of medical marijuana and you consent to treatment with medical marijuana while agreeing to comply with Chapters 2925 and 3796 of the Revised Code and this division. OGT will assemble a recommendation profile and submit to the Patient Registry on your behalf.

Upon review and approval by the State Board of Pharmacy, you will be invited to create an account on the Patient Registry. Once completed, you will be required to submit a $50 payment directly to the Board so you can be placed in the Patient Registry and receive your Medical Marijuana ID Card. The ID card will be in digital format but you have the option to print out should you prefer. Your Medical Marijuana ID Card allows you to purchase medicine from Ohio-based dispensaries!
